I'm having problems with my rear wheel coming off. I make sure that it's on super tight but it won't stay on. And it's not a case of the Quick Release lever coming open. It's still in the closed position. Is this a problem with the QR itself and I should just replace the skewer?

Check to make sure there's no dirt in there.
If it's clean you should get a new qr assembly, should fix it up.
I anm guessing that it is not a Shimano qr? Some of the DT, Mavic etc. lower end qr's are pretty shitty. I had a DT on my Nomad that would become kinda loose pdq as did my buddies. Replaced with a proper Shimano QR and problem solved. You need the small cam steel qr. Not the ones with the larger diameter cam made from aluminum with or without the plastic bearing.

I'd just get a Shimano QR if you haven't like Oldfart says. Others may be pimpier but they are generally crap. I can't see how putting washers in there will help unless you've got a bizarre set up like a 125mm rear end and a QR designed for a 135mm rear end?
If you happen to be in down town, we've probably got various ones thrown in a drawer and you could have one. I'm not saying where I work as I don't believe in using the forums for subtle advertising (unless I get paid for it….ahem.) but you can always PM if you desire.

we always recommend them for kiddies running horizontal dropout'd, single-speed DJ frames who have issues with the back wheel moving when using a nice shiny Hope quick release skewer
